Behavioral & Mental Health (BMH) Community Care Navigator (RN)

Key Responsibilities

The BMH Community Care Navigator coordinates the placement and admission of behavioral and mental health patients to ensure clinically appropriate levels of care. They also facilitate care coordination across hospital, clinic, and home settings while collaborating with interdisciplinary teams to optimize patient flow and outcomes.

Requirements

Candidates must have at least 5 years of RN experience, including 2 years in critical care or emergency nursing, and hold a valid Georgia nursing license. A Bachelor of Science in Nursing and certifications such as CCM or PMH-BC are preferred.
