Key Responsibilities

The Physical Therapist will plan and administer personalized rehabilitation programs for patients within their homes to improve strength and mobility. They are also responsible for documenting patient progress and coordinating care with physicians, the clinical manager, and the broader care team.

Requirements

Candidates must hold a current and unrestricted Physical Therapy license and possess valid CPR certification. Experience collaborating with a team of healthcare professionals is required, with one year of physical therapy experience preferred.
