Social Worker LMSW (Weekends Required)

Key Responsibilities

The Social Worker provides direct services to patients and families, including mental health assessments, crisis intervention, and discharge planning. They coordinate referrals to community resources, nursing facilities, and home health services while maintaining accurate documentation in the Cerner system.

Requirements

Candidates must hold a Master’s degree in Social Work and possess a current LMSW license in the state of Texas. A minimum of two years of hospital-based social work experience is required for this position.
