The Mental Health Counselor is responsible for providing psychosocial assessments, collaborative treatment planning, engagement, referrals, and evidenced-based clinical interventions as indicated for individuals dealing with a variety of mental health and co-occurring substance use issues. 

As part of the Essential Function for this role, the Mental Health Counselor:   

* Conducts initial psychosocial assessments and provides initial treatment recommendations, and collaboratively with the patient develops a personalized treatment plan with emphasis on evidence-based practice within a harm reduction orientation

* Provides resources and referrals as indicated to facilitate achievement of a patient’s treatment goals

* Provides psychotherapy and crisis intervention to individuals and families who are dealing with a variety of issues, including but not limited to mental health, co-occurring substance use disorders, chronic medical conditions, including HIV, HCV, and issues related to sexual orientation and gender identity

* Adheres to program expectations and requirements surrounding productivity, documentation timelines and standards, and overall quality of patient care

* Works within an integrated treatment model, coordinating treatment with both internal and external providers as indicated

Qualified Candidate will have a Masters of Social Work or other related field with two (2) years of related experience; LMSW or LMHC required, LCSW preferred; candidate must be able to communicate clearly and professionally in writing and verbally, in English (with additional Spanish-speaking strongly preferred); demonstrate familiarity with computers and MS Office software (Outlook, Word) and effective time management and organization skills; awareness of and sensitivity to the issues and health disparities faced by underserved populations is essential. 

Job Type: Full time, 40 hours (hybrid/flexible) 

Required Education: Master’s degree in social work (or similar); LMSW or LMHC required, LCSW preferred

Required Experience: Two (2) years of relevant experience; knowledge of Diagnostic and Statistical Manual of Mental Disorders; working knowledge utilizing electronic medical records software (Medent) strongly preferred. 

This position is posted with a salary range. The actual rate of pay offered will be based on a variety of individualized factors, including but not limited to a candidate’s skills, relevant experience, educational background, internal equity, and market considerations.
