Overview The Care Team is seeking a Registered Nurse Case Manager to join our growing team! The Registered Nurse Case Manager supports hospice patients receiving care in their homes or facility settings by providing skilled nursing care and coordinated, compassionate end-of-life support in partnership with the interdisciplinary team. Schedule, Hours & Territory

  • Standard operating hours are Monday through Friday, 8:00 AM to 4:30 PM
  • Territory: Up to a 1 hour radius of Carmel, IN
  • RN Case Managers may participate in an on-call rotation with additional pay
  • Some branches offer dedicated on-call staff, resulting in backup on-call coverage only What We Offer

  • Conduct comprehensive patient assessments and develop individualized plans of care
  • Provide skilled nursing care in accordance with hospice standards and physician orders
  • Coordinate care with physicians, caregivers, and interdisciplinary team members
  • Educate patients and families on symptom management, disease progression, and end-of-life care
  • Document assessments, interventions, and care plans accurately and timely in the EMR HCHB (Homecare Homebase)
  • Participate in interdisciplinary team meetings and care planning
  • Support after-hours patient needs as part of an on-call rotation when applicable Qualifications:
  • Active Registered Nurse (RN) license
  • Hospice or home health experience preferred
  • Strong clinical assessment and care coordination skills
  • Ability to work independently in the field
  • Strong communication and documentation skills
  • Experience using HCHB (Homecare Homebase) preferred Field Role Requirements
  • Active driver’s license
  • Ability to pass a criminal background check
  • Ability to pass a Motor Vehicle Record (MVR) check
  • Current TB testing in accordance with agency policy
  • Current CPR certification as required for the role

Requirements

Candidates must hold an active Registered Nurse (RN) license and possess strong clinical assessment and documentation skills. Experience in hospice or home health settings is preferred, along with the ability to work independently in the field and pass background and motor vehicle checks.
