Position Summary
We are seeking a mission-driven, compassionate, and forward-thinking Bilingual in Vietnamese Primary Care Physician (PCP) to join our best-in-class, full-risk Value Based Care Primary Care Practice Group. This role is designed for a physician who thrives in a value-based care environment and wants to focus deeply on relationship-based medicine, proactive health management, and personalized patient engagement.

Key Responsibilities
Patient-Centered Clinical Care
- Deliver comprehensive, longitudinal primary care to a panel of Commercial and Medicare age patients
- Provide patient- centered appointments, allowing time for whole-person care, counseling, education, and shared decision-making.
- Conduct Annual Wellness Visits (AWVs), preventive screenings, and evidence-based chronic care visits in alignment with CMS and other health plan quality metrics.
- Develop and maintain personalized care plans based on patient goals, risk factors, and social determinants of health.
- Integrate behavioral health, social support services, and care management into care plans in collaboration with the interdisciplinary team.
Care Team Collaboration for Value-Based Performance & Quality
- Drive quality performance by proactively identifying and closing care gaps aligned with HEDIS, STARS, and preventive screening measures.
- Deliver accurate, complete, and compliant documentation following CMS HCC risk adjustment guidelines and MEAT criteria to ensure optimal RAF scoring.
- Review and act upon pre-visit planning tools, post-visit summaries, and quality dashboards supported by the MSO/IPA.
- Coordinate with Care Management, Pharmacy, Behavioral Health, and Social Work teams for complex patient needs and transitions of care.
Ideal Candidate Profile
Education & Licensure
- MD or DO from an accredited medical school.
- Board Certified/Board Eligible in Family Medicine or Internal Medicine.
- Active and unrestricted license in the state of California.
- Current DEA and NPI registration.
Experience
- 2–3 years of outpatient primary care experience preferred (Internal or Family Practice); senior-focused or complex care highly preferred.
- Prior exposure in Medicare Advantage, ACO, or Full-Risk VBC environments strongly preferred.
- Experience managing chronic disease populations with multiple comorbidities.
- Strong foundation in quality metrics (HEDIS/STAR), HCC risk coding, and care team leadership is preferred

Requirements
Candidates must hold an MD or DO degree and be board certified or eligible in Family or Internal Medicine with an active California license. A minimum of 2-3 years of outpatient primary care experience is preferred, along with proficiency in Vietnamese and Korean.
