About the Role

Key Responsibilities
- Perform resident assessments to determine level of care within facility
- Develop and implement a plan of treatment and aftercare plans for the resident and/or family, including obtaining collateral information from family and support network
- Evaluate resident progress and revise treatment plan as needed
- Provide knowledgeable of community resources available for residents and families
Requirements
- Master’s degree in social work, MHC, Psychology, Public Administration, or similarly related field
- LMHC or LCSW certification or license eligible
- At least two (2) years of experience in a Therapist/Counselor position working directly with residents is preferred
- Must have superior interpersonal skills and be proficient in delivering individual, group, family therapy and case
