Key Responsibilities

Perform resident assessments to determine level of care within facility

Develop and implement a plan of treatment and aftercare plans for the resident and/or family, including obtaining collateral information from family and support network

Evaluate resident progress and revise treatment plan as needed

Provide knowledgeable of community resources available for residents and families

Requirements

Master’s degree in social work, MHC, Psychology, Public Administration, or similarly related field

LMHC or LCSW certification or license eligible

At least two (2) years of experience in a Therapist/Counselor position working directly with residents is preferred

Must have superior interpersonal skills and be proficient in delivering individual, group, family therapy and case
