Job Description:
Make a Difference Beyond the Clinical Visit
Elara Caring is hiring a part-time Medical Social Worker to support home health patients and families throughout Portland and surrounding southern Maine communities.
This is meaningful work. You will help patients navigate emotional, social, financial, and environmental challenges that may affect their ability to remain safely at home. You will work independently in the community while receiving support from an experienced interdisciplinary team.

What You’ll Do
Assess patients’ psychosocial, financial, and environmental needs
Complete initial social work evaluations and follow-up visits
Provide counseling, education, and support to patients and caregivers
Connect families with community programs and financial resources
Participate in developing and updating individualized Plans of Care
Collaborate with physicians, nurses, therapists, and other care team members
Communicate changes or concerns affecting patient safety and well-being
Support discharge planning and continuity of care
Complete accurate and timely clinical documentation
What You’ll Bring
Master’s or Doctoral degree in Social Work from a program accredited by the Council on Social Work Education
Current Maine Social Worker license in good standing
At least one year of social work experience in a healthcare setting
Strong communication, counseling, documentation, and advocacy skills
Ability to work independently in patients’ homes
Willingness to travel throughout the assigned service area with 50% travel required
Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations
