Director of Rehabilitation — Physical Therapy

As Director of Rehabilitation, you'll lead the physical therapy program while remaining clinically engaged. You'll develop your team, elevate clinical reasoning, build relationships across the community, and partner with leadership to make rehabilitation an integral part of the resident experience.
You'll
- Lead and develop an exceptional therapy team
- Maintain an active clinical caseload
- Drive clinical quality, outcomes, and evidence-informed practice
- Build strong relationships with residents, families, caregivers, nursing, and community leadership
- Develop programming around mobility, balance, fall prevention, strength, function, and independence
- Support residents across Assisted Living, Memory Care, and wellness programming
- Identify opportunities for early intervention and proactive clinical support
- Build strong referral and interdisciplinary relationships
- Use data and clinical insight to continually improve the program
