About the Role

Join Everent Health & Affiliates as a Primary Care Physician, PACE Services in United States. Care Guide Partners PACE is a part of Hosparus Health. As a Program of All-Inclusive Care for the Elderly (PACE) provider, we work to empower people 55 and older with the support they need to stay in the homes they know and love as they age.

Requirements

  • Valid MD license in good standing to practice medicine in the State of Kentucky.
  • Fellowship or additional training in Geriatrics preferred.
  • Minimum two years clinical experience required, with experience working with frail or elderly populations strongly preferred.
  • Excellent communication skills, with ability to interact with a wide range of stakeholders, including the IDT Team and external healthcare providers.

Work Schedule

  • Full-time position.
