Key Responsibilities

• Provide supervised hospice and palliative medicine care as an integrated member of the hospice interdisciplinary team across inpatient, outpatient, and home- based hospice settings.

• Perform comprehensive symptom assessment and management for patients with serious and life-limiting illness, including pain, dyspnea, nausea, anxiety, depression, and delirium.

• Participate actively in hospice operations, including longitudinal patient assignments, interdisciplinary team (IDT) meetings, and coordination of care across disciplines.

• Conduct goals-of-care discussions, advance care planning, and end-of-life conversations with patients and families.

Work Schedule

• Full-Time position.
