As a Physical Therapist, you will provide in-home evaluations, treatments, and education to help patients improve strength, mobility, safety, and independence. You will work closely with nurses, occupational therapists, speech therapists, and other members of the interdisciplinary team to ensure comprehensive, patient-centered care.   • Conduct comprehensive in-home physical therapy evaluations
  • Develop and implement individualized treatment plans focused on mobility, strength, balance, and safety
  • Provide therapeutic interventions including exercise programs, gait training, transfer training, and pain management
  • Educate patients and caregivers on home exercise programs, safety strategies, and equipment use
  • Collaborate with nurses, occupational therapists, speech therapists, social workers, and case managers to coordinate care
  • Document all patient visits, progress notes, and care plans in compliance with agency and regulatory standards
  • Recommend adaptive equipment and home modifications when appropriate
  • Monitor patient progress and adjust treatment plans as needed
  • Communicate changes in condition or concerns to the interdisciplinary care team

## Qualifications Licensed to practice as a Physical Therapist in the state of Iowa. Minimum of two years of experience in physical therapy; community/home health experience preferred. Strong communication, organizational, and problem-solving skills. Valid driver’s license with reliable transportation.

Key Responsibilities

Conduct comprehensive in-home physical therapy evaluations and develop individualized treatment plans to improve patient mobility, strength, and safety. Collaborate with an interdisciplinary team to coordinate care and document patient progress in compliance with regulatory standards.

Requirements

Must be a licensed Physical Therapist in the state of Iowa with at least two years of experience. A valid driver's license and reliable transportation are required for home visits.
