Make a Real Difference Every Day – Company Car with Gas Card
Are you a compassionate Licensed Clinical Social Worker who wants to provide truly meaningful support to patients and families at the end of life? At Companion Hospice, our Medical Social Workers are essential members of the interdisciplinary team—helping patients and families navigate the emotional, practical, and psychosocial challenges of a life-limiting illness with dignity, comfort, and support.
If you’re looking for a role where your clinical skills, empathy, and resourcefulness truly matter, this is it. We’re offering a company car with gas card (no monthly charge to you), along with the opportunity to make a lasting impact in one of the most meaningful areas of healthcare.
Why Social Workers Love Working Here
- Build deep, rewarding relationships with patients and families during a critical life transition
- Work in a supportive, collaborative Interdisciplinary Group (IDG) environment
- Enjoy strong autonomy with excellent clinical and leadership support
- Focus on quality of life rather than curative treatment—hospice is about living as fully as possible
- Serve your local community (Guthrie, Edmond, Stillwater, and surrounding areas) with a family-owned, locally operated organization
Your Impact as a Hospice Medical Social Worker
- Conduct in-depth psychosocial assessments to identify the emotional, social, financial, and community needs of patients and families
- Provide short-term counseling, anticipatory grief support, and crisis intervention
- Collaborate with the IDG to develop and update the Plan of Care
- Connect patients and families with community resources, assist with Medicaid applications, and support advance directives and POA/DPOA issues
- Participate in bereavement support as needed and complete timely, accurate documentation
- Contribute to discharge planning, continuous quality improvement, and staff education
- Perform on-call duties as required
What You’ll Bring
- Master’s degree in Social Work (MSW) from a CSWE-accredited program
- Licensed Clinical Social Worker (LCSW) in Oklahoma (required)
- At least one year of experience as a social worker; hospice or home care experience preferred
- Knowledge of local community resources preferred
- Excellent communication, interpersonal, and organizational skills
- Valid driver’s license and current auto insurance
- Ability to work independently in patients’ homes and community settings while remaining a strong team collaborator
- Commitment to the hospice philosophy of care
What We Offer – Real Benefits That Matter
- Company car + gas card (we do not charge you monthly fees)
- Competitive pay (exempt position)
- Supportive interdisciplinary team and leadership
- Meaningful, mission-driven work focused on quality of life
- Opportunity to serve patients and families in their homes, assisted living facilities, and nursing homes across the Guthrie/Edmond/Stillwater area
Physical Demands & Work Environment (summary)
This role involves regular travel to patient homes and community locations in varying weather conditions, intermittent standing/walking/stooping, lifting up to 20–25 lbs, and the ability to document and communicate effectively. Reasonable accommodations may be available.
