Key Responsibilities

• Evaluate, diagnose, and treat a broad range of acute and chronic conditions.
• Collaborate with clinical and operations teams to ensure continuity of care.
• Ability and willingness to provide multi-state coverage (additional licensure a plus).

Requirements

• Active, unrestricted NP or Physician license in multiple U. S. states.
• Board Certification in Family practice.
• In compliance with federal law, all persons hired will be required to verify identity and eligibility
• to work in the United States and to complete the required employment eligibility verification document form upon

Work Schedule

• Providers must be available for evening and weekend coverage and commit to a minimum of 15 hours per week.
• Weekends: 9:00 AM – 12:00 PM
• Minimum commitment: 15 hours per week
• Preferred availability: Evenings and weekends
• Flexible scheduling within clinical hours
• Prescribe medications and order labs/imaging as clinically appropriate.
