Who You Are The Enhanced Living Caseworker is responsible for promoting the resident’s well being and satisfaction by supporting resident centered care, developing and interpreting standards of care, guiding the interdisciplinary team as it establishes and implements the resident’s plan of care (RASP) with the resident and his/her family or responsible party and the neighborhood team.

What You'll Do

Perform admissions, transfers, and discharges within the Enhanced Living program and provides assistance to residents and their families as physical, social and emotional problems or needs arise. Welcome residents to the neighborhood and provide orientation of programs, other residents, and team members.

Responsible for the initiation and maintenance of a resident’s RASP through collaboration with the resident, family, and neighborhood team while promoting person directed choices within the life of the neighborhood. Ensure implementation of the RASP with neighborhood team members.

Ensure regulatory compliance, including encouraging appropriate record keeping and documentation in neighborhoods Provide support to the neighborhood managers modeling quality of life enhancement practices and serve to support the values of person-centered, elder-directed care.

Participate as a team member to address level of care changes within the Admissions meetings Potential Career growth track: Social Worker What Else You'll Need High School Diploma/GED equivalency required.

Bachelor’s degree from an accredited school in social work preferred Minimum of two (2) years’ experience in a Senior Living setting preferred Knowledge of casework principles and social work methods Knowledge of Personal Care Home regulations Maintains a current and valid Pennsylvania driver’s license Qualifications
