Requirements

  • Doctor of Medicine (MD/DO) Completed a three-year Family or Internal Medicine Residency.
  • Active medical license in the state of Ohio.
  • Minimum of 2 years of experience as a Provider in an Outpatient Primary Care Practice or Urgent Care.
  • Electronic Medical Record experience.
