Key Responsibilities

The Social Worker will assess hospice patients to identify psychosocial, financial, and community needs while providing counseling and crisis intervention. They are responsible for facilitating community resources and ensuring compliance with Medicare regulations and clinical documentation standards.

Requirements

Candidates must hold a Master’s degree in social work and a current state license (LMSW required, LCSW preferred). At least one year of clinical experience is required, along with a valid driver's license and reliable transportation.
