Occupational Therapist Per Visit, Home Health Care / Western Nassau / Long Island

Key Responsibilities

The Occupational Therapist provides safe, therapeutic, and age-appropriate occupational therapy to restore or improve functional physical abilities, promote cognitive/behavioral changes, and adapt surroundings for optimal daily living tasks (ADL/IADL). Responsibilities include comprehensive patient evaluation, establishing measurable goals, implementing and modifying treatments, collaborating with the healthcare team, and maintaining accurate documentation.

Requirements

Candidates must possess a Bachelor's Degree from an ACOTE-approved NYS OT program and hold a current, valid NYS Occupational Therapy license and registration, along with Basic Life Support Certification. A minimum of 1-2 years of experience in a Hospital (Inpatient), Orthopedic, Outpatient, or Pediatric Rehabilitation setting is required.
