Key Responsibilities
The Hospice Social Worker performs psychosocial, emotional, and bereavement assessments for patients and families. They also develop and implement goal-directed care plans while serving as an advocate for the patient within an interdisciplinary team.
Requirements
Candidates must hold a Master's Degree in Social Work and an active New York State Social Work license. Prior experience in a community health, hospice, or inpatient setting and a valid driver's license are required.
