  • Evaluate patients using subjective data, objective measures, clinical assessment, prognosis, and function‑based goals, establishing effective and individualized treatment plans.
  • Implement and modify patient‑specific treatment programs to support functional progress and long‑term outcomes.
  • Develop and deliver patient and family education, adjusting strategies as needed to maximize engagement and understanding.
  • Document all clinical findings, plans, and progress in the electronic medical record in a timely and accurate manner.

Must be a graduate of an accredited Physical Therapy program with a current or pending New Hampshire license. Experience in outpatient physical therapy and CPR certification are required.
