Provide one-on-one physical therapy to older adults in their homes or senior living communities using evidence-based treatment plans. Collaborate with an interdisciplinary team and utilize telehealth tools to support patient mobility, independence, and wellness.
Must hold a valid Physical Therapy license or be eligible to obtain one and possess a degree from an accredited Physical Therapy program. Candidates must be proficient with EMR systems, hold current CPR certification, and be physically capable of performing in-home patient care.
