H2 Health is seeking an Occupational Therapist to support our outpatient clinics in Bluefield, Virginia, and Tazewell, Virginia. In this regional opportunity, you'll work with a diverse outpatient population while delivering individualized, evidence-based care focused on restoring function and improving quality of life. You'll collaborate closely with Physical Therapists, physicians, and other healthcare professionals while treating patients with orthopedic, neurological, post-surgical, upper extremity, and functional impairments. This position offers the flexibility of full-time or part-time employment while providing the opportunity to build meaningful patient relationships across two community-focused outpatient clinics.

Your Role

Perform comprehensive occupational therapy evaluations and develop individualized plans of care.

Deliver evidence-based interventions designed to improve functional independence, strength, coordination, mobility, and activities of daily living.

Treat patients with orthopedic, neurological, post-surgical, upper extremity, and functional impairments.

Collaborate with patients, caregivers, physicians, and interdisciplinary team members to achieve optimal outcomes.

Educate patients and caregivers on adaptive strategies, home programs, and techniques to improve daily function and independence.

• Maintain accurate and timely documentation of patient progress and treatment outcomes.
• Utilize Scribe AI-powered documentation technology to streamline documentation and maximize patient care time.
• Utilize the Raintree EMR platform for documentation, scheduling, and patient management.
• Contribute to a positive clinic culture focused on patient-centered care and clinical excellence.
• Degree in Occupational Therapy from an ACOTE-accredited Occupational Therapy program.
• Current Virginia Occupational Therapist license or eligibility for licensure.
• CPR certification or willingness to obtain as required.
• Strong communication and interpersonal skills.
• Commitment to providing high-quality, patient-centered care.
