Key Responsibilities

  • Review prescriptions issued by prescribers to assure accuracy and determine formulas or ingredients needed.
  • Evaluate prescriber medication orders for appropriateness of drug, dosage, potential interactions, route of administration and stability. Make recommendations for medication therapy changes to prescribers as appropriate.
  • Collaborate with the healthcare team to provide clinical and technical support as needed.
  • Document all communication with physicians, nurses, and other members of the healthcare team.

Requirements

  • Graduate of ACPE accredited School of Pharmacy.
  • Current licensure in the state of practice.
  • Long-term care pharmacy experience preferred.
  • Experience in patient care and medication management, drug therapy monitoring, drug and medical supply procurement, inventory control management, and performance improvement.

Work Schedule

  • Full-Time position.
  • No call required.
  • No weekends required.
