The Physical Therapist will perform home health visits to provide rehabilitative care, diagnostic testing, and individualized treatment plans for adult and geriatric clients. They are responsible for documenting care, educating families, and collaborating with a multidisciplinary team to promote client health and safety.
Candidates must hold a current VA Physical Therapist license and have at least one year of recent work experience. Graduation from an accredited physical therapy program is required, along with strong organizational and computer skills.
