Fee-for-Service Physical Therapists Are Required To
- Work a minimum of 2 shifts per pay period (every 2 weeks)
- Attend the full first week of orientation, Monday through Friday from 8:30 AM – 4:30 PM
If no prior CHHA experience, commit to training 2 days per week until fully oriented and independently managing
Job Details
Our Home Care PTs
- Review patient charts and medical history
- Develop a personalized treatment plan
- Teach therapeutic exercises and stretches to improve patient’s conditions
Use Hands-on Therapy to Promote Healing
- Maintain patient records by keeping track of goals and progress while adapting treatment plans accordingly
- Work closely with a supportive patient care team including physicians, nursing, social services, pastoral care, home health aides and others for your assigned caseload.
- Provide patient and family education for health promotion as directed by physician’s plan of treatment.
- Provide care to the most upmost standard
Minimum Qualifications
New York State PT License
- Minimum of one year of recent hands on experience as a Physical Therapist.
- Valid New York State Driver’s License and reliable transportation
Salary Range
USD $62.00 - USD $135.00 /Hr.
This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job, and it does not include the value of benefits.

Key Responsibilities
The Physical Therapist will develop and implement personalized treatment plans for home care patients, including therapeutic exercises and hands-on therapy. They will also maintain accurate patient records and collaborate with an interdisciplinary team to ensure high-quality care.
Requirements
Candidates must hold a valid New York State Physical Therapist license and have at least one year of recent hands-on experience. A valid New York State driver's license and reliable transportation are also required.
