Speech-Language Pathologist (SLP) – Home Health
Senior Helpers of Melbourne

Senior Helpers of Melbourne is expanding our skilled home health services and is looking for a compassionate and dependable Speech-Language Pathologist to join our growing clinical team.
We are looking for an SLP who enjoys working directly with patients in their homes and is comfortable providing independent, patient-centered care in a home health setting.
Position Summary
The Speech-Language Pathologist will provide skilled speech therapy services to patients in their homes in accordance with physician orders, the patient's plan of care, and applicable home health regulations.
The SLP will evaluate and treat patients experiencing communication, cognitive, speech, language, voice, and swallowing difficulties and work closely with patients, caregivers, physicians, and the interdisciplinary care team.
Responsibilities
* Perform comprehensive speech-language pathology evaluations in the patient's home.
* Evaluate speech, language, communication, cognitive, voice, and swallowing function as clinically appropriate.
* Develop individualized treatment plans based on patient needs and goals.
* Provide skilled therapy for speech, language, cognitive-communication, and swallowing disorders.
* Provide dysphagia treatment and swallowing education when indicated.
* Educate patients and caregivers regarding treatment strategies, safety precautions, exercises, and techniques that can be continued between visits.
* Monitor patient progress and modify treatment plans as clinically appropriate.
* Communicate changes in condition, concerns, and progress with the physician, Director of Nursing, and interdisciplinary care team.
* Complete required clinical documentation accurately and within required timeframes.
* Participate in care coordination and interdisciplinary communication as needed.
* Follow all agency policies, infection-control procedures, professional standards, and applicable state and federal requirements.
Qualifications
* Current and unrestricted Florida license to practice Speech-Language Pathology.
* Graduate degree in Speech-Language Pathology from an accredited program.
* Certificate of Clinical Competence (CCC-SLP) preferred.
* Current CPR/BLS certification.
* Valid driver's license, reliable transportation, and ability to travel to patients' homes throughout the service area.
* Home health experience preferred, but not required.
* Experience with adult and geriatric patients preferred.
* Experience evaluating and treating dysphagia and cognitive-communication disorders preferred.
* Strong clinical judgment, communication, organization, and documentation skills.
* Comfortable working independently in a home-based clinical environment.
* Must meet all applicable background screening and employment requirements.
