Key Responsibilities

• The Home Health team delivers patient-centered rehabilitation services in the home setting, supporting patients in regaining function and independence where they live.

• You will evaluate patients in their homes, develop individualized occupational therapy plans and goals, and communicate findings and progress with patients, families, physicians and the care team.

Requirements

• Bachelor's Degree in Occupational Therapy required.

• Completion of internship program in Occupational Therapy.

• Working knowledge of standard treatment and evaluation tools required.

• Licenses and Certifications.

Benefits & Compensation

• Salary: $104,603 - $135,990
