Key Responsibilities

The Physical Therapist will manage a caseload of patients, conducting comprehensive in-home assessments and delivering skilled therapy visits. They are responsible for developing and executing personalized plans of care while coordinating with the multidisciplinary team to ensure positive patient outcomes.

Requirements

Candidates must hold an active Michigan Physical Therapist license and possess strong clinical judgment for independent field work. Prior home health experience is strongly preferred, along with excellent communication skills and the ability to travel daily.
