Provide in-home physical therapy services to help patients restore function, relieve pain, and prevent disability. Collaborate with physicians and interdisciplinary teams to develop and implement individualized treatment plans.
Requires a Bachelor's degree in Physical Therapy, a valid state license, and BLS certification. Candidates must have 1-2 years of PT experience, including recent Home Health experience and proficiency in HCHB and OASIS.
