We are seeking an Advanced Practice Provider to join one of the nation’s most integrated healthcare systems, with over 1,300 providers in Indianapolis, IN. This opportunity is located at our Stop 11, Family Medicine Practice, at 5908 E. Stop 11 Road Indianapolis, IN 46237.

Community Health Network has an excellent opportunity for a family medicine trained Advanced Practice Provider to work at our outpatient Stop 11, Family Care Medicine practice.

Experience

Family Medicine experience as a Nurse Practitioner is preferred

Must successfully complete a nationally recognized Advanced Practice Nurse certification examination or have current national certification; must maintain certification on an ongoing basis.

Must be eligible for prescriptive authority under the criteria outlined by the Indiana State Board of Nursing; must maintain eligibility for prescriptive authority on an ongoing basis.

Key Responsibilities

Provide comprehensive family medicine care to patients across all age ranges within an integrated healthcare system. Utilize the EPIC electronic medical record system to manage patient health and contribute to population health management initiatives.

Requirements

Must be a nurse practitioner with a nationally recognized certification and maintain eligibility for prescriptive authority in Indiana. Previous family medicine experience is preferred.
