Prava Therapy is seeking a full-time Speech-Language Pathologist to join our team within a vibrant Independent Living community in Apex, North Carolina.

Your Role
As a Speech-Language Pathologist, you'll provide skilled clinical care addressing the complete scope of adult and geriatric speech-language pathology while becoming a trusted partner within the community.
You'll
Perform comprehensive evaluations and develop individualized, evidence-based plans of care across all adult/geriatric SLP domains.
Assess and treat disorders in cognitive-communication, language (aphasia), speech production/voice (dysarthria/apraxia/dysphonia), swallowing/dysphagia, pragmatic communication, and functional AAC/environmental memory supports.
Target functional medication management, calendar use, financial tasks, and daily routine organization from a cognitive-linguistic perspective.
Educate residents, families, caregivers, nursing, and dining staff on communication strategies, cognitive support tools, safe swallowing protocols, and dietary modifications.
Collaborate with physicians, ENT specialists, community leadership, dining services, wellness professionals, OT/PT partners, and other members of the care team.
- Monitor progress and modify treatment plans based on clinical presentation and functional outcomes.
- Maintain timely, accurate, and compliant documentation.
- Participate in care coordination, interdisciplinary team conferences, and discharge planning as appropriate.
- Contribute to a culture of clinical excellence, compassion, professionalism, and continuous learning.
