The Behavioral Health Navigator provides outreach, engagement, and care coordination for individuals with or at risk of Hepatitis C. They help clients navigate complex medical, mental health, and social service systems to ensure continuity of care and improved health outcomes.
Requires a high school diploma or GED and two years of experience in outreach, peer support, or case management. Candidates must have an understanding of recovery principles and experience working with diverse populations, particularly those impacted by substance use and trauma.
