Work Schedule
- Full-Time position.
- On-site position.
- No weekends required.
This position offers weekends off. Work with University of Rochester in Rochester, New York.
Key Responsibilities
Responsibilities include the evaluation and management of acute and chronic illnesses, ordering and interpreting diagnostic tests, conducting physical exams, and managing hospital follow-ups within a Patient Centered Medical Home model. The APP will also collaborate with the medical team, facilitate specialist referrals, provide patient education, and participate in the education of various learners.
Requirements
Candidates must be a Nurse Practitioner (NP) or Physician Assistant (PA) possessing required NYS certification and licensure, and must have applied for DEA registration prior to the start date. An NP with over 2 years of family medicine practice experience is favored, though less experienced APPs with strong primary care passion will be considered.
