Job Description
Codman Square Health Center is a Federally Qualified Health Center serving a diverse community in Dorchester, MA, and surrounding neighborhoods. CSHC offers a full continuum of services such as primary care, urgent care, behavioral health, care management, nutrition, family planning, optometry, dentistry, laboratory, pharmacy, radiology, and medical specialties including HIV care, substance use treatment, Hepatitis C, OB-GYN, Nephrology, and Cardiology alongside an on-site STI clinic.

Position Summary
The Registered Dietitian/Nutritionist (RDN)is responsible for helping patients and community members improve their health and well-being. The RDN will provide Medical Nutrition Therapy (MNT) to patients across the lifespan, from diverse backgrounds through telemedicine and in person visits.
The RDN may counsel individual patients or their families in one-on-one or group settings.
Other responsibilities may include teaching nutrition and wellness classes, leading and/or overseeing cooking demonstrations, creating, and maintaining nutrition education materials, co-facilitating group visits, developing community programming, and promoting worksite wellness among employees.
The RDN interacts with the medical, nursing, and social staff on developing patient care plans which are effective and culturally appropriate. The RDN promotes patients’ self-management and empowers patients/families to achieve maximum levels of wellness and independence.
Primary Responsibilities
Patient Care
- Provide culturally relevant medical nutrition therapy and nutrition counseling to patients from diverse backgrounds.
- Provide counseling services to patients via in-person visits, warm handoffs from providers, and telehealth.
- Provide both one-on-one care and conduct group visits and classes.
- Document all patient visits using the Nutrition Care Process to specify goals for patients with varying medical diagnosis such as diabetes, obesity, and heart disease.
- Participate on multi-disciplinary teams to optimize patient care.
Nutrition Programs
- Develop culturally appropriate education materials for patient use to increase the knowledge of nutrition for diverse populations.
- Provide nutrition programming within the Codman Square community to expand public awareness of nutrition, health, and disease prevention.
- Provide nutrition resources and support to partner agencies including Codman Academy, and the school-based health clinics at Tech Boston High School and Randolph High School.
- Act as a preceptor to graduate students and dietetic interns from local universities.
CSHC Staff and Patient Engagement
- Develop and perform staff and patient education and marketing regarding the Nutrition Department. Allow students and residents from a variety of disciplines to shadow nutrition visits to learn more about the department.
- Collaborate with CSHC staff to leverage internal communications to promote the Nutrition Department with patients, staff, and providers.
- Facilitate “warm hand-offs” by participating in real-time conversations about nutrition services with patients immediately after they receive a referral from a provider.

Requirements
Qualifications & Skills
- Bachelor’s degree in dietetics, foods and nutrition, or related area, granted by a US regionally accredited college or university, or foreign equivalent.
- Master’s degree preferred.
- Completion of required dietetics coursework and supervised practice through a Didactic Program in Dietetics and Dietetic Internship, or a Coordinated Program in Dietetics accredited by the Accreditation Council for Education in Nutrition and Dietetics (ACEND) of the Academy of Nutrition and Dietetics.
- Experience in outpatient nutrition counseling.
- Group counseling experience preferred.
- Experience working with low-income populations and people from diverse backgrounds.
- Ability to build new relationships and communicate effectively among a wide array of individuals.
- Bilingual in Spanish or Haitian Creole a plus.
- Experience using Epic a plus.
- Registered Dietitian/Nutritionist (RDN) by the Commission on Dietetic Registration (CDR).
- Massachusetts licensure as dietitian/nutritionist Registered Dietitian/Nutritionist.
Physical Requirements
- Must be able to stand or sit for prolonged periods (at least 50% of the time)
- Ability to lift up to 25 pounds and load onto shelves.
- Visual acuity sufficient for frequent reading and computer use
