Responsibilities

Evaluate patients and develop personalized care plans focused on functional independence, safety, and quality of life

Deliver care through therapeutic activities, ADL training, and adaptive techniques

Help patients improve their ability to perform daily tasks such as dressing, bathing, grooming, and meal preparation

Partner with interdisciplinary teams, caregivers, and nursing staff to support strong patient outcomes and continuity of care

Track patient progress, maintain accurate clinical documentation, and adapt treatment plans based on patient needs and rehabilitation goals

Qualifications

  • Licensed Occupational Therapist (OT) or license eligible in the state of practice
  • Graduate of an accredited Occupational Therapist Program
  • Interest or experience in skilled nursing, long-term care or geriatric rehabilitation
  • Passion for patient-centered care and improving quality of life for older adults
