About the Role

The Roads Home Health Care is seeking a Home Health Medical Social Worker (MSW) to provide psychosocial assessment and support to patients and families throughout South Florida. In this role, you'll help patients and their loved ones navigate the emotional, practical, and financial challenges that come with illness or a life-changing diagnosis, connecting them with the resources they need to cope and thrive at home. Both full-time and per diem opportunities are available.

• Perform comprehensive psychosocial assessments in patients' homes
• Obtain and communicate physician orders for certification and recertification of medical social work services
• Coordinate care planning with physicians, patients, families, and the interdisciplinary care team
• Educate patients and families on available resources and coping strategies
• Deliver social work interventions appropriate to each patient's needs and clinical status
• Identify safety, environmental, and mobility risks in the home and recommend interventions
• Connect patients and families with community resources to support optimal outcomes
• Support transfer and discharge planning in accordance with agency policy
• Complete timely, accurate documentation and participate in interdisciplinary team conferences
• Supervise social work assistants and support the team in delivering coordinated patient care

Job Requirements

• Current, valid Florida clinical social worker license
• Master's or doctoral degree from an accredited school of social work
• Minimum one year of post-master's experience in a clinical setting
• Current CPR/BCLS certification, current health certificate, and TB clearance
• Valid driver's license, reliable vehicle, and current auto insurance; ability to pass a criminal background check

Qualifications

• Strong clinical judgment and ability to synthesize assessment data into an effective care plan
• Excellent interpersonal and communication skills for building rapport with patients and families
• Ability to work independently while managing a home visit schedule
• Compassionate, culturally sensitive approach to patient and family support
• Strong organizational and documentation skills
• Genuine commitment to advocating for patients and connecting them with the support they need
• Prior home health experience preferred
• Bilingual candidates a plus
