Key Responsibilities

The Clinical Nurse Navigator provides direct patient-facing nursing care, clinical triage, and patient education within an ambulatory primary care setting. They manage clinical work queues and coordinate care across multidisciplinary teams to ensure high-quality patient outcomes.

Requirements

Candidates must hold an Associate's degree or higher in Nursing and possess a current, valid Registered Nurse license in the District of Columbia. A minimum of two years of clinical nursing experience is required, with preference given to those with ambulatory or primary care backgrounds.
