Job Description
Overview
The Heart Failure and Pulmonary Hypertension Nurse Practitioner utilizes a patient-centered coordinated care model, demonstrating competencies in leadership, direct clinical practice, consultation/collaboration, coaching/guiding, research, and ethical decision-making. The APN works collaboratively with the practice/hospital team to assess, plan, and implement care for individuals with health and safety needs.
The NP position is dedicated to the comprehensive care of patients with pulmonary hypertension. The APN will manage this specific patient population across both inpatient and outpatient settings, ensuring continuity of care and specialized treatment.
Core Clinical Duties and Privileges
- Manage a daily inpatient assignment average of 5-10 patients
- Manage patients in the outpatient clinic setting, including new patient evaluation
Formulating daily plan of care including current management of medical therapy with IV inotropes, IV prostacyclins, discharge plan, and collaborating with multidisciplinary team
Interpret Clinical data, hemodynamic monitoring including right heart catheterizations and Swan-Ganz catheter hemodynamics to guide treatment of Pulmonary Hypertension.
Operational and Scheduling Requirements
Provide coverage and support providers for both inpatient and outpatient services as scheduled.
Work schedules are 8 hour shifts, which may include rotating weekends. Outpatient clinic hours are typically standard business hours.

- Maintaining Pulmonary Hypertension Accreditation- obtained- 8/2024
- Collecting data for the Pulmonary Hypertension Program for organization and research.
- Developing PAH education and lectures for nurses and providers
- Serve as a liaison and collaborate with specialty pharmacies on IV, SQ and oral medication for PH patients
Responsibilities

Collaborates with medical staff to support nursing-led protocols. 22. Mentors other APNs, precepts students in APN programs and participates in educational programs for nursing as a clinical expert. 23. Identifies patient/family educational needs and collaborates to implement appropriate teaching materials. 24. Facilitates and participates in nursing educational programs as a clinical expert. 25. Promotes a collaborative work environment supporting the organizational mission, behavior expectations, values, and pillars. 26. Promotes innovation by integrating new clinical knowledge into practice. 27. Participates in clinical research activities and disseminates findings through practice, education, presentations, and publications. 28. Identifies ethical care issues and coordinates interdisciplinary meetings to resolve them. 29. Adheres to the legal framework and scope of practice defined by the NJ Di

Qualifications
Education, Knowledge, Skills and Abilities Required: 1. Graduate of an NLN/AACN accredited nursing program. 2. Master's or Doctorate degree from an accredited Nurse Practitioner program. 3. Maintains clinical competencies through continuing education and clinical practice. 4. Experience in a relevant patient care setting (or as regulations dictate). 5. National Provider Identifier (NPI). 6. Prescriptive authority through CDS and DEA registration according to New Jersey state law. 7. Collaborative agreement with physicians on staff. 8. Excellent written and verbal communication skills. 9. Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms. BLS AHA Basic Health Care Life Support HCP Certification. 4. Board certified by AANP or ANCC. 5. AHA Advanced Cardiac Life Support Certification. 6. Acute Care NP cert.
