About the Role

Join Mercyhealth Wisconsin and Illinois as a Behavioral Health Case Manager, Autism Program - FT in United States.

Key Responsibilities

Assists in assessing participants for continuing treatment needs, prepares patient histories, discharges, and creates treatment plan of care.

Assists in assessing patient needs and develops appropriate dispositional plans and coordinates care with the appropriate departments.

Maintains strong communication and relationship with staff (interdepartmental and within program) and Psychiatrists.

Hourly rate: $39.68/hr.

A Bachelor's Degree in a mental health or behavioral health field is required, with working toward a higher degree or licensure being preferred.
