Job Description
Medical Social Worker - Licensed
Purpose of Position
Provide psycho-social assessments and social services to people of all ages. Counsel and aid patients and/or families to help the patient understand and accept medical recommendations. Assess and treat non-medical issues interfering with effective patient care. Work on a medical unit and provide discharge planning, and possibly group therapy.

Essential Functions
* Provide medical social work services to patients of all ages, including the patients located in the Acute Care Unit, Primary Care Clinic and village clinics via telephone, walk-in patients and to staff.
* Provide child and adult protective services assistance and consultation to staff and patients.
* Respond to requests for medical social work services, including case management, discharge planning, nursing home/assisted living placements, risk man agement, crisis intervention, care coordination, resource identification and response to referrals made by physicians, nurses or administration.
* Coordinate nursing home and assisted living placements for patients.
* Provide medical social work services, including assessments, participation in care conferences, case management, discharge planning, etc. to the residents of QCC in the absence of the Social Services Director.
* Assist in locating community resources for patients.
* Advocate for patient or resident rights issues and actively support the Corporation’s philosophy by participating in patient/customer focused care activities and quality assessment/improvement team activities.
* Perform other duties as assigned.

* Knowledge of legal aspects of protective services, legal representation options for patients, confidentiality and patient/resident rights.

Requirements
Requires a Master's degree in Social Work or a related field and at least two years of experience in medical social work or protective services. Candidates must obtain an Alaska Social Worker License (LMSW or LCSW) within six months of hire.
