Occupational Therapist, Home Health

The Occupational Therapist provides high-quality, patient-centered care by managing the complete process of occupational therapeutic care of an assigned group of patients, in coordination with the physician and the patient care team.

The Occupational Therapist completes patient assessments, and participates in developing and implementing individualized care plans, focusing on goal achievement, patient engagement, and positive patient experience.

  • 1 Year Home Health Experience
  • Bachelor of Science in Occupational Therapy from an accredited program
  • Successful completion of the National Certification Examination for Registered Occupational Therapist
  • State license in practicing state
  • Reliable transportation and proof of valid automobile liability insurance
  • Must have valid driver’s license
  • Health Care Plan (Medical, Dental & Vision)

Key Responsibilities

The Occupational Therapist manages the complete therapeutic care process for assigned patients in a home health setting. They are responsible for conducting patient assessments and developing individualized care plans in collaboration with the medical team.

Requirements

Candidates must hold a Bachelor of Science in Occupational Therapy and possess a valid state license. A minimum of one year of home health experience and a valid driver's license with insurance are required.
