This position qualifies for federal student loan forgiveness. After 10 years of qualifying payments while working full-time for a nonprofit employer, the remaining balance on your Direct Loans may be forgiven.

Join St. Paul's Senior Services as a PACE Social Worker in United States.

Key Responsibilities

Provides direct medical social work case management to a designated caseload of TRU PACE participants and families.

Completes initial and ongoing reassessment of participant and family psychosocial needs based on emotional, environmental, financial, psychological, cognitive, social, cultural and health factors.

Collaborates with the interdisciplinary team, participant and family in the development, implementation, evaluation, and revision of the comprehensive care plan.

Evaluates quality of psychosocial interventions and adjusts interventions as clinically necessary TRU's.

Requirements

Candidates must possess a Master’s Degree in Social Work and have at least two years of relevant experience as a medical social worker. Experience working with frail or elderly populations, individuals with mental health needs, and within an Interdisciplinary Team model is preferred.
