Long Term Contract for Outpatient Physical Therapist in Colebrook, NH with $2700 Weekly Gross Weekly Gross

Sage Group HealthCare Staffing is seeking a Physical Therapist for a travel contract position in Colebrook, NH. We are accepting both local and travel candidates.

Position Details:

• Pay Rate: Range $2720 to $2800 weekly Gross Pay based on a 40-hour work week (Includes Non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)
• Unit: Hospital based outpatient clinic
• Shifts available: Days
• Shift timings: 8 hours, Days, 8.00 am to 4.30 pm

Schedule: 40 hours/week guaranteed, 5 X 8-hours, Monday-Friday, No weekends, No call

Position Requirements

• Physical Therapist experience: 2+ years - Required
• PT Outpatient experience - Required
• PT Inpatient experience - Preferred
• Prior Travel experience – Preferred

Education, License, and Certification Requirements

• PT License for the State applying or Compact if applicable - Required
• BLS Certification - Preferred

Job Description

• Provides all aspects of PT Services in a hospital based outpatient clinic and occasionally inpatient.
• The physical therapist will accurately assess, differentially diagnosis, and identify deficits in a patient’s ability to move and perform functional activities in their daily lives.
• Develop individualized treatment plans and performs evaluations.
• Establishes an individualized plan of care and selects treatment techniques that addresses the specific impairments identified in the evaluation and in accordance with physician orders.
• Conducts ongoing reassessment of the patient and their response to treatment.
• Completes legible, accurate and clear documentation within the established timeframe
• Assesses educational needs of the patient, family, caregiver, or significant other and provides education to meet those needs.
• Functions within scope of practice as defined by the state’s practice act.
• Performs other related duties as required and directed.
