About the Role

Join FlexStaff as a Hospital Social Worker – Queens Health System in United States. FlexStaff is seeking Social Workers with various levels of experience to work for one of our clients, the largest city health system for their location in Queens, NY. This is a permanent, full-time job offering benefits.

Key Responsibilities

Conduct rapid psychosocial assessments for patients and families facing acute medical conditions, trauma, or end

Provide crisis intervention, brief counseling, and emotional support to help patients and families cope with illness, injury, and complex medical decisions.

Lead and coordinate comprehensive discharge planning, ensuring safe and timely transitions to post

acute care, home, or other appropriate settings.

Requirements

Schedule: 8:00AM

Salary: $85,078 Social Worker Level 3:

Requirements: LCSW

4:00PM or 9:00AM

Full-Time position.

Day shift only - no nights.
