How You’ll Contribute

Coordinates and provides pharmaceutical care for patients, collaborating with the interprofessional team to optimize safe, effective medication use. Reviews and verifies orders, manages drug therapy protocols, educates staff/patients, and supports quality and regulatory standards. Reports to the Director or Manager of Department.

Essential Functions

  • Manages drug therapy protocols as part of an interdisciplinary team.
  • Reviews, interprets, and evaluates medication orders to identify, prevent, and resolve drug-related problems.
  • Provides drug information and education to medical staff, healthcare professionals, patients, and pharmacy students.
  • Documents interventions and outcomes for drug therapy quality improvement.
  • Verifies medication orders for accuracy/completeness prior to entry; checks filled medications prepared by technicians; maintains proficiency with pharmacy automated systems and software.
  • Manages patients in an outpatient anticoagulation clinic, including point-of-care testing, result evaluation, dosing, and scheduling.
  • Participates in performance improvement and required continuing education.
  • Performs other duties as assigned.

Qualifications and Requirements

Education: Doctor of Pharmacy.

License: Current license with the state Board of Pharmacy.

Certifications: BCLS required; ACLS preferred. Skills: Critical thinking, decisive judgment, and ability to work with minimal supervision in stressful environments. Rapid learning and navigation of multiple pharmacy/clinical software systems.

STRHS-Lawrenceburg, formerly Crockett Hospital, is a 99-bed, acute care facility located in Lawrenceburg, Tennessee, 85 miles south of Nashville.

We have been providing the best healthcare in Lawrence County and surrounding counties in Southern Middle Tennessee since 1974. We expanded the facility and services in 1991 with the addition of an Outpatient Surgery Unit, a state-of-the-art 24-hour physician-staffed Emergency Department, and an Intensive Care Unit.

The hospital opened a 10-bed Physical Rehabilitation Unit in 1997, and a Women's Health Center in 1999. Diagnostic imaging services include in-house MRI, Nuclear Medicine, CT, X-ray, Digital Mammography and Ultrasound. The Rehabilitation Unit provides Physical, Occupational and Speech Therapies.

In 2009, a $5.6 million construction was completed which expanded the Emergency Department, Outpatient Surgery Unit and Registration area.

Key Responsibilities

The pharmacist coordinates and provides pharmaceutical care by collaborating with an interprofessional team to optimize medication use. They are responsible for reviewing medication orders, managing drug therapy protocols, and educating staff and patients.

Requirements

Candidates must hold a Doctor of Pharmacy degree and a current license with the state Board of Pharmacy. A BCLS certification is required, while ACLS certification is preferred.
