Social Worker - Hospice (Full-Time) Salary: $65,000 - $81,309.97 Position Summary: Provide for the psycho-social and bereavement needs of assigned hospice patients and bereaved family caregivers while collaborating closely with members of the interdisciplinary team.

Key Responsibilities

Assess needs of patients and families and develop a plan of care in collaboration with health care team to optimize psychosocial functioning of patients and families

Facilitate patient and family participation in treatment activities

Implement intervention strategies and referrals to appropriate community resources to ensure continuity of care, patient and family satisfaction and quality services

Provide timely feedback and follow
