Social Worker - Hospice (Full-Time) Salary: $65,000 - $81,309.97 Position Summary: Provide for the psycho-social and bereavement needs of assigned hospice patients and bereaved family caregivers while collaborating closely with members of the interdisciplinary team.
Key Responsibilities
- Assess needs of patients and families and develop a plan of care in collaboration with health care team to optimize psychosocial functioning of patients and families
- Facilitate patient and family participation in treatment activities
- Implement intervention strategies and referrals to appropriate community resources to ensure continuity of care, patient and family satisfaction and quality services
- Provide timely feedback and follow
