Position Summary
As an NHC Registered Dietitian, you have a vital and valued role in our patients’ lives. You will assess patients’ nutritional needs; develop, implement, and monitor care plans; and ensure compliance with federal, state, and center guidelines. Success in this role requires strong communication, food systems management knowledge, and interdisciplinary collaboration.
Responsibilities
- Complete and document admission, quarterly, annual, significant-change, and high-risk patient nutrition assessments
- Develop and monitor individualized nutrition care plans
- Provide nutrition education and training to food service and center staff
- Collaborate with nursing, therapy, administration, and food and nutrition services to promote optimal patient outcomes
- Participate in care plan meetings and interdisciplinary committees
- Lead Quality Assurance and Performance Improvement (QAPI) initiatives, conduct audits, and implement action plans for continuous improvement
- Complete MDS assessments and documentation
Qualifications
- Bachelor's or Master's degree in Nutrition or Dietetics from an accredited college or university
- Registered Dietitian Nutritionist (RDN) credential through the Commission on Dietetic Registration (CDR)
- Current RD licensure
- Knowledge of long-term care regulations and nutritional standards
- Strong organizational, communication, clinical, and interpersonal skills
- Basic computer and documentation skills
Schedule: In-person, Monday–Friday (80 hours bi-weekly), with flexibility within the week.
