Physical Therapist (PT)

We are seeking compassionate, dependable, and motivated Physical Therapists to provide skilled physical therapy services to patients in their homes.

Responsibilities

• Perform comprehensive physical therapy evaluations and assessments.
• Develop individualized treatment plans based on patient needs.
• Provide skilled therapeutic interventions to improve mobility, strength, balance, coordination, and functional independence.
• Monitor patient progress and modify treatment plans as appropriate.
• Educate patients and caregivers regarding exercises, mobility, safety, and fall prevention.
• Collaborate with physicians, nurses, PTAs, and other members of the interdisciplinary care team.
• Complete accurate and timely clinical documentation.
• Provide professional, compassionate, patient-centered care in the home setting.

PT Requirements

• Graduate of an accredited Physical Therapy program.
• Current and active Florida PT license.
• CPR certification.
• AHCA Level II background screening.
• Home health experience preferred.
• Strong clinical, communication, and organizational skills.
• Ability to work independently.
• Reliable transportation.

Physical Therapist Assistant (PTA)

We are seeking a dependable, compassionate, and motivated Physical Therapist Assistant to provide quality therapy services to patients in their homes under the direction and supervision of a licensed Physical Therapist.

Responsibilities

• Provide physical therapy treatments according to established plans of care.
• Assist patients with therapeutic exercises, mobility, balance, strength, and functional activities.
• Monitor patient response and progress during treatment.
• Educate patients and caregivers on prescribed exercises, mobility, safety, and fall-prevention techniques.
• Report changes in patient condition or progress to the supervising Physical Therapist.
• Maintain accurate and timely clinical documentation.
• Collaborate with the Physical Therapist and interdisciplinary care team.
• Provide professional, compassionate, and patient-centered care in the home setting.

PTA Requirements

• Graduate of an accredited Physical Therapist Assistant program.
• Current and active Florida PTA license.
• CPR certification.
• AHCA Level II background screening.
• Home health experience preferred.
• Strong communication and organizational skills.
• Ability to work independently and manage a flexible schedule.
• Reliable transportation.
