Job DetailsJob Location: Melissa Memorial Hospital - HOLYOKE, CO 80734Position Type: Full TimeEducation Level: Graduate DegreeSalary Range: $64,116.80 - $100,182.50 SalaryJob Shift: DayJob Category: Health CareJOB SUMMARY: The Licensed Behavioral Health Counselor is responsible for providing behavioral health consultation to adults and families in a fast-paced primary care setting, utilizing a multidisciplinary team approach, to improve psychosocial functioning.

Responsible for providing behavioral health assessment, consultation, crisis intervention and brief treatment services to promote adherence to individualized care plans and assist patients in managing symptoms and underlying stressors.

This position will work primarily in the outpatient setting; however, may also assist in the ER and inpatient setting.

ESSENTIAL DUTIES: Demonstrates competency using an evidence-based approach conducting behavioral health assessments, goal setting and providing effective treatment in the least restrictive and appropriate setting. Employs effective clinical interview techniques that facilitate development of a therapeutic relationship.

Provide patient centered treatment and make appropriate referrals to and coordinates with the interdisciplinary team and community resources to supplement treatment as needed.

Counsels’ individuals/groups via face to face, telephone, or electronic modality regarding psychological or medical concerns including but not limited to chronic disease burden, stress, substance misuse, and/or family situations which impede adherence to care plan.

Develops and implements a therapeutic treatment plan in a medical setting and provides education to the individuals/groups to promote knowledge, understanding, and effective management of behavioral health problems and psychiatric disorders.

Ability to thrive in a busy clinical environment with frequent interruptions and maintain a calm, reassuring and empathic demeanor. Receive real time consultations of patients in the ambulatory setting who need BHC services.

Displays competency in assessment, evaluation and management of crisis situations and application of appropriate interventions as needed. Documents all care, progress toward goals and treatment plans accurately and in a timely manner as per policy and procedure and according to regulatory mandates.

Able to prioritize workload appropriately, collaborate as a member of the patient care team, and clearly and succinctly communicate assessment, diagnoses, progress, and treatment plan to teams.

Manage panel of patients to ensure continuity of care, patient adherence to care plans, and identification of barriers preventing adherence to care plan. Actively participate in transformational development activities that support Core Physicians' integrated behavioral health program.

Educate and assist the primary care team in developing guidelines, and education for patients to improve self-management of chronic disease. Incorporates data and information to continuously improve care and practice to enhance outcomes. Regular and predictable attendance.

Continuous learning in both personal and professional development.

Consistent review, comprehension and reply of company email and related correspondence. *In addition to the responsibilities described above, the role may include other responsibilities and duties as assigned on occasion, based upon MMH needs or requirements. Maintain confidentiality of all work information. Demonstrate an ability to function successfully in a team environment. Exhibit courteous, compassionate and respectful treatment of internal and external customers. Displays a positive attitude and flexibility in changing situations.

Participates in identifying problems and suggesting solutions. Report any compliance issues as designated in the MMH Compliance Program.

Qualifications

QUALIFICATIONS: MINIMUM: Required education: Licensed Clinical Social Worker (master’s with License) or Licensed Professional Counselor Preferred Education: Licensed Clinical Social Worker (master’s with License) or Licensed Professional Counselor with experience working in an outpatient clinic or inpatient setting.

Skills: Analytical, organized, multitasked, excellent communicator, collaborative, relationship building, scheduling experience.

PREFERRED: Preferred Education: Licensed Clinical Social Worker (master’s with License) or Licensed Professional Counselor with experience working in an outpatient clinic or inpatient setting. Bilingual but not required.

It is a model rural hospital with 1 operating room, surgical procedure room, 4 bay emergency room, and radiology including 64-slice CT scanner and mammography, MRI, ECHO, and Nuclear Medicine, along with other ancillary services. The hospital is co-located with a family practice clinic and retail pharmacy services.

Other services include chemotherapy, general & orthopedic surgery, podiatry, cardiology, orthopedic surgery, progressive rehab services, and wound care. MMH operates a full-time EMS service and maintains a 24/7 AirLife Helicopter base.

THE COMMUNITY: Holyoke is home to the Holyoke Dragons and is truly a “City of Pride and Progress.” Holyoke offers: From different coffee shops to different ethnic flavors, to a quick bite, the sweet small town of Holyoke has a handful of different restaurant options to choose from.
