Provide advanced social work practice and psychosocial assessments for transplant services patients and their families. Collaborate with an interdisciplinary team to manage social, emotional, and economic concerns to maintain optimal patient functioning.
Requires a Master's degree in Social Work and a valid professional license in Minnesota or North Dakota. Experience in healthcare or mental health hospital settings is preferred.
